These strays, or ‘Stray Omegas’, who fall through the cracks of the management system, cannot receive suppressants and are unable to take regular jobs. Most of them, it’s said, end up working as sex workers in red-light districts.

They turn the Omega trait of heat into a means of survival.

“Can they pay for the treatment?”

“The person accompanying them said they’ll pay in cash.”

Without insurance, the cost, including the initial consultation fee, would be high. But as long as they can pay in cash, the hospital has no issue. Some doctors might shoo away such cases because they don’t want to deal with the hassle outside normal hours. But Kazusa wasn’t the type to turn away someone injured simply because they were a Stray Omega.

“I see.”

He glanced again at the intake form. The patient’s address was listed as Deep Downtown, age 20, male. The name was—

“…Serval?”

Stray Omegas, lacking a national number, also lack an official registered name with the National Registry Bureau. Most of their parents are also strays, so they have no concept of family names.

Thus, most Stray Omegas live their lives under a nickname given by their parents.

(Still, couldn’t they have chosen a more normal name?)

“Please bring them in.”

The nurse nodded and left the room, returning a short while later.

“Please come in.”

At her prompting, a young man entered the examination room.

As expected, it was immediately obvious he was an Omega. His blonde hair, cut in a straight line, had pink streaks running through it. His thin, arched eyebrows framed large, slanted, cat-like eyes. His nose was slim and slightly upturned at the tip, and his lips were thin. His face came to a sharp point.

He appeared to be about 170 cm tall. He was wearing a stylish white shirt with intricate designs, paired with black skinny jeans that had a distressed look, and black sneakers. Though he wasn’t particularly tall, his well-proportioned figure made him appear slender.

Regardless of gender, Omegas are known for their exceptional beauty, as if they were crafted by the gods to be admired. Because of this, many of them thrive in the entertainment industry as idols, models, actors, singers, or dancers. Their heightened sense of creativity also leads some to excel as artists, painters, writers, composers, and in other creative fields.

Their striking looks and sensitivity are both natural gifts. Kazusa understood how challenging life could be for Omegas, burdened by the monthly heats. But as a plain Beta himself, he couldn’t help but wonder if he might have led a more exciting life had he been born ‘special’ like them.

“Please, have a seat.”

Kazusa became aware again of his own ensnarement by the curse of lookism as he prompted Serval, who still had a boyish air about him, to sit down. He waited for Serval, who was cradling his left hand, to take a seat on the stool before rotating his office chair to face the patient.

Upon closer inspection, the patient’s skin was radiant. And apart from his injury, he appeared healthy. Kazusa had treated Stray Omegas from red-light district brothels several times before. Many of them, having been physically overworked since their mid-teens when their heat cycles began, suffered from various ailments. Stray Omegas were generally known to have short lifespans. Their beauty was often accompanied by a fleeting, fragile quality, much like a mayfly’s brief existence.

However, the young man in front of him didn’t exude that sense of fragility. If he were to be categorised into light and shadow, he would definitely be on the side of light.

(He’s a rare type.)

“What brings you in today?”

Though this was a doctor’s routine question, Serval readily lifted his left arm and said, “I hurt myself.” He pointed to a spot about five centimetres from his wrist, where a temporary bandage had been wrapped. There was no blood seeping through, indicating that the bleeding had been successfully stopped.

“How did the injury happen?”

“Well… after dinner, I tried to slice a peach with a paring knife. Then my hand slipped and… before I knew it, blood was gushing out.”

“I see. So, it wasn’t intentional but an accident?”

“Yes! I—um… it wasn’t self-harm or anything.”

Kazusa didn’t respond to the pleading look Serval gave him but simply said, “Let me take a look at the wound.”

At Kazusa’s request, Serval began to remove the bandage. Although it looked a little difficult for him, Kazusa didn’t offer assistance. He let Serval handle it. Slowly and awkwardly, the young man revealed the fresh wound.

Kazusa gently supported Serval’s left arm, carefully examining the wound. It was a clean cut, over five centimetres long and about 1.5 centimetres deep, located on the outer part of his forearm. It seemed to match the story about the paring knife.

“Did you rinse the wound with running water?”

“The person who’s with me washed it with tap water.”

The person with him, presumably the one waiting in the reception area to pay the bill, must have been knowledgeable about first aid. The bleeding had been perfectly stopped, and having someone familiar with emergency care nearby had been fortunate for Serval.

“That’s the right procedure. Rinsing the wound with running water helps reduce the bacteria, lowering the risk of infection. Can you bend your left arm?”

Serval nervously but slowly bent his left arm.

“Does it hurt when you move it?”

“It stings a little.”

“Now, try bending your fingers one by one. Take your time.”

As instructed, Serval folded each of his five fingers, starting with his thumb.

“Good. Now, try opening them again… Okay. No issues with your nerves or tendons.”

At Kazusa’s diagnosis, Serval let out a sigh of relief.

“However, the wound is wide open. I can see the subcutaneous fat. It would be best to stitch it up.”

Serval’s expression tensed at once.

“You mean… you’re going to sew it up? Will it hurt?”

“I’ll administer a local anaesthetic, so there won’t be any pain during the procedure. Stitches will speed up the healing process and help the wound heal neatly. Since it’s in a visible area, it would be better if the scar isn’t too noticeable, right?”

“…Yes.”

Taking Serval’s response as consent, Kazusa instructed the nurse, “Prepare the anaesthesia, please.”
